Disability Community Vaccination Event will be held for persons with disabilities, caregivers, children 5 years old and older, and any community members living in the greater Phoenix metropolitan area.
Available at this event:
– Booster
– Pfizer
– Moderna
– Johnson & Johnson
Get 1st Shot, 2nd Shot or Booster
Saturday, February 12 | 3PM to 8PM
Saturday, March 5 | 3PM to 8PM
Must be 5 years old or over. Minors must have an Authorized Health Representative for consent. Vaccinations will be administered in a walk-in clinic setting at the Ability360 Sports & Fitness Center gymnasium. ADA reasonable accommodations and American Sign Language (ASL) will be provided upon request in the sign-up system. An appointment is required.
Transportation
The AHCCCS Health Plans are providing transportation to vaccination. Area Agency on Aging will provide Free transportation to vaccine shots for individuals 60 years or older in Maricopa County. Contact Derek Oetken 602-241-5562.
