Position Summary

Provide support to the Home Care Services Unit (HCS) through active engagement of contracted health plans. Act as Liaison between health plans and Ability360 HCS on a variety of issues including communication, awareness, quality care, referral development, and retention.

Program Service

Ability360 delivers high-quality, person centered non-medical home care to AHCCCS/ALTCS/DDD eligible members in their homes or places of residence, including attendant care, personal care, homemaker, respite, and habilitation services.

Required Education and Qualifications

  • Combination of eight (8) years education and experience in home care services working directly with AHCCCS/ALTCS health plans.
  • Experience with the Department of Developmental Disabilities a plus.
  • Experience working with health plan case managers and case management teams.
  • Bachelor’s Degree preferred.
  • Existing relationships with health plan leadership highly desirable.
  • Broad knowledge of Arizona non-medical home care regulations and services agreements.
  • Outstanding communication skills in both small and large group settings.
  • Ability to influence change through value proposition and services excellence.
  • Skilled at gaining support from both provider and health plan leadership/decision makers.

Essential Responsibilities / Accountabilities

  • Conduct regular face-to-face meetings with health plan representatives documenting attendees and discussions around referral volumes, action items, issues, and resolutions.
  • Participate in health plan meetings and updates, community forums, and provider engagement activities.
  • Meet regularly with HCS supervisors and managers to address opportunities, solicit feedback, and share information.
  • Regularly provide health plans with Ability360’s available workforce by service type and geography.
  • Share with health plans information about Ability360 events and comprehensive services.
  • With support from the Vice President, manage and report on Value Based Reimbursement initiatives with designated health plans.
  • Participate in strategic planning meetings and provide subject matter expertise in relation to health plans.
  • Provides back up to referral coordinator during vacation and sick leave
  • Other duties and special projects as assigned.
  • Personal experience with and/or knowledge about disability preferred.

Work Environment

While performing the duties of this job, the employee regularly works in an office setting. Local travel by car, 50%.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.
