Our Reintegration to Community program works with individuals who are newly injured or diagnosed in hospitals or rehabilitation centers, and individuals who are living in nursing homes and long-term care facilities and want support to reintegrate and return to Community life.
Our Community Reintegration program helps participants overcome barriers to living in the Community. Our Reintegration to the Community Coordinator will connect you to local resources, peer support networks, identify housing resources, support in obtaining assistive technology, equipment, or modifications to enhance your independence. These services help reduce institutionalization, promote independence, and improve quality of life by ensuring that each individual has the tools, supports, and confidence to live fully and activity within their community.
